General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: ...the hazards of broken bones and crushed feet. Employee(s) working in the main foundry cleaning area were exposed to the hazard of fractures, broken bones, and other foot injuries when working with a Shepard Niles, electric powered wire rope hoist. Two (2) strands of the 3/8" hoist rope were broken. The hoist is used to raise steel castings weighing 150 - 200 pounds each. A feasible method, among other methods, to abate this condition can be accomplished by removal of the damaged cable segment, reattachment of the cable to the lifting hook using an approved technique, inspection and certification of the repaired hoist by a competent person. In affecting repair, not less than two full wraps of the cable shall remain on the hoist when the hook is lowered to its maximum extension, a load attached, and a lift undertaken. If all of the above conditions cannot be met, the entire lifting cable shall be replaced. DISCLAIMERS: 1) You are not limited to the abatement methods suggested above. 2) The methods explained are general and may not be effective in all cases. 3) The employer is responsible for selecting and carrying out an appropriate abatement method.
