Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
falling, crushing, & pinch point injuries.
a) Falling Injuries:
1) From the bin floor of the feed mill, through openings at the
southwest and southeast corner of the hoistway for the electric
passenger elevator car.
2) From the first floor to the basement of grain elevator structure,
through the freight elevator hoistway opening while the elevator
car is being lowered to the basement floor.
b) Crushing Injuries:
1) From being struck by the descending electric passenger elevator
car in the Feed Mill.
2) From being struck by the descending electric freight elevator in
the grain elevator structure.
c) Pinch Point Injuries:
1) From having a portion of the body between the electricpassenger
elevator car, and the floors, hoistway gates or supports around
the elevator hoistway at the seed cleaner, bin & headhouse levels
of the feed mill.
2) From having a portion of the body between the electric passenger
elevator car and the floors, hoistway gates or guardrails around
the elevator hoistway at the ground, bin, and distributor
(headhouse) levels of the grain elevator structure.
Among other methods, one feasible and acceptable abatement method to
correct these hazards is to take the following steps:
1) Enclose the elevator hoistways for the two passenger elevators and
the freight elevators to a height of at least 7' above each landing
level.
2) Guard each elevator hoistway landing opening to the full height and
width of openings at each entrance for both passenger elevators and
the freight elevator.
3) Insure that the hoistway doors for both passengr elevators and the
freight elevator are equipped with hoistway unit system hoistway
door interlocks.
4) Insure that the two passenger elevator cars are equipped with
door/gate electric contacts to prevent the elevator car from moving
unless the door or gate is closed.
