Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a)  Transamerican Natural Gas Corporation Rig Number 75;
M.V. Herbst Number 13 in Zapata County South of Laredo, Texas
off of Highway 83 the hazard of derrick and/or mast failure
due to improper welds during construction and/or stress during
normal operations shall be prevented by insuring that all
welds on support members of the derrick and/or mast are
maintained to withstand the weight of employees and other
stresses which may be placed on them such as specified by API
RP54 First Edition January 1981, Page 16, Paragraph 8.2.13.
This alleged condition occurred on or about October 16, 1991.
