Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of fire and explosion: (a) On or about December 31, 1991, an explosion occurred when sparks created by an employee using an abrasive grinder to remove a coupling from a piping system (metal) containing hydrogen gas ignited the hydrogen gas, resulting in an explosion. Among other methods, one feasible and acceptable abatement method to correct this hazard is to include all spark producing grinding operations in hazardous or potential hazardous locations into the company's existing hot work permit program.
