Section 5(a)(1) of the Occupational Safety & Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
of serious physical harm to employees in that employees were exposed to:
a)  The employees working as burners on the baling machine were exposed
to serious injuries from inadvertant activation of the dump cycle when
cutting the overhanging steel straps or other material while atop either
of the two feed hoppers.  The hoppers (-the large one on the west side
and the small one on the north) could not be locked out by the burners
prior to going onto the hopper and any inadvertant activation of the hopper
would result in the shifting of the steel or possibly knocking the employee
off the platform.
Among other methods, one feasible and acceptable method of abatement to
correct this hazard is to have a separate electrical lockout device
installed at each of the burners' stations.  The burners must then be
instructed to lockout the feed hopper that they must enter to do the
torching operations so that the hopper cannot be inadvertantly activated.
