General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employee(s) were exposed to the hazards of oil and gas well blowouts, fire, and/or derrick collapse. Employee(s) working at various levels of a well service rig were provided only one means of egress, the derrick ladder, use of which could be blocked by the preceding hazards. A secondary emergency only, means of egress to remove the employee(s) from the proximity of the hazards could limit their exposure. Among other methods to provide a secondary means of egress, one feasible and acceptable means would be the installation and expeditious use of a properly rigged and secured "Geronimo" line. An employee was exposed to a well blowout and subsequent fire on or about July 30, 1990, at the Roberson N 6-11 well site, rural south west of Oklahoma City, Oklahoma.
