Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
Employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
a)  On or about October 9, 2000 and prior to that time, employees who
performed boring
operations with a Ditch Witch 410 SX Vibratory Plow were exposed to caught
in hazards of the
rotating solid stem boring rod attachments, due to employees either hand
holding or foot
controlling the bore direction of the boring rods.
Among other abatement methods to correct this hazard, one acceptable
method is to assure
boring operations are performed in accordance with the manufacturer's
recognized practices
which include the following:
1)  Keep employees at least 10 feet away from turning drill string and do
not straddle the trench
or drill string while boring.
2)  Utilize a rod guide (T-Bar) if needed to direct the initial bore path
rather than hands or feet
to guide the rotating boring rods, which could expose loose clothing to be
caught in the rotating
rods.ing
