Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On or about February 7, 1992 employees preparing to hand load
cement into a concrete truck hopper while being elevated in the bucket
of Cat Loader SN 62K9144 were exposed to hazards of approximately 9 to
10' falls due to being thrown from the bucket or falling from the
bucket. In addition, they were exposed to the potential of being struck
by falling bags of cement.
Among other methods, feasible and acceptable abatement methods to
correct these hazards  are the use of appropriately guarded stairways,
work platforms, scaffolds, aerial lifts  or other means which would
provide both safe access and fall protection.
