Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
On or about February 6, 1991; employees performing
maintenance and repair in the vacuum brazing furnace
were exposed to severe burn hazard due to the potential
ignition of magnesium dust.
Among other methods, one feasible and acceptable method to correct
these hazards would include;
1) Establish and implement a written "hot work" permit system that
defines the conditions and safeguards necessary to grind, weld or
perform other heat/spark producing work in the vacuum brazing
furnace. Salient features of the program would include, but not be
limited to:
a) Inspection of the area where work is to be performed.
b) Establishment of fire watches, when required.
c) Providing manned fire extinguishing equipment, by a standby
employee.
d) Communication and coordination between all employees/departments
concerned.
e) Isolation of all combustibles and flammables including coatings.
f) Limiting the use of flame, heat or spark producing equipment.
2) Train employees in the recognition and avoidance of hazards
associated with this work.
