Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards tha there causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Warehouse shop area 1-employees were exposed to crushing hazards from
falling loads in that a 1 ton "jet" chain hoist was used to lift the
front axle of a Caterpillar Model TD 50 D SA Forklift Truck, serial
number NPB 914, which has an empty weight of 3450 pounds on the front
axle.
Among other methods, one feasible and accepted method to correct this
hazard is to install and use hand chain hoists in accordance with the
manufacturing instructions and/or in accordance with the American
National Standards Institute safety standard for overhead hoists (ANSI
B30.16-1989) Paragraph 16-3.1.3(h) of ANSI B30.16.1989 which
specifically prohibits overloading of hand chain hoists.
