Section 5(a)(1) of the Occupational Safety and Health Act of 1970:    The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that:
(a)  On or about January 26, 1995 employees were exposed
to the hazard of being struck by the American horizontal
boring machine due to operation without the roller hook
guards securing the boring machine to the tracks.
Among other methods, one feasible and useful method to
correct this hazard is to develop, implement and enforce a
work rule which requires the attachment of the boring
machine to the base tracks with roller hooks as required by
the manufacturers installation/operation manual and as
stipulated in American National Standard Institute B11.8
"Safety Requirements for Construction, Care, Use of
Drilling, Milling, and Boring Machines, 1983 as follows:
ANSI B11.8
Section 3.2.4 Hazards to Personnel Associated with
Tipping or Falling Machines.  The machine itself shall be
designed, where necessary, with provision to secure the
machine to the floor or to a bench or machine stand.  The
manufacturer shall indicate in the installation instructions
the conditions under which the machine should be secured
to the floor or foundation.

Page 6, Setting and Aligning the Track and Installing the
Machine.  Set the machine on the master track and engage
the push bar dogs in the rearmost holes in the track.  Install
the slide rails or clamp hook rollers, then lower the master
casing pusher.
