Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The portable, stand-mounted, Ridgid pipe threader, Model 400A, used by Machinery Maintenance Department personnel, was operated without the use of a foot switch or other momentary contact device to control the power input to the machine, as prescribed by the American National Standard Institute , ANSI B208.1-1982, Section 4.4. Employees operating the threader without a foot switch were exposed to being caught and dragged into the rotating parts of the machine.
