Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On or about January 27, 1995 employees were exposed
to the hazard of being struck by hoist suspended loads due
to use of an overhead hoist with a kinked and frayed load
line.
Among other methods, one feasible and useful method to
correct this hazard is to replace this cable and to implement
a program of running rope inspections as prescribed in
ANSI B30.16, 1973 American National Standard Institute,
standard for overhead hoists as follows:
16-2.2.4.1  Inspection
(a)All running ropes in periodic use throughout the
shift shall be visually inspected once each shift by
the operator or an appointed person. A thorough
inspection of all ropes shall be made at least once
each month and a full written, dated and signed
report of rope condition kept on file where readily
available to appointed personnel.  All inspections
shall be performed by an appointed or designated
person.  Any deterioration resulting in appreciable
loss of original strength, such as described below,
shall be carefully noted and determination made as
to whether further use of the rope would constitute
a safety hazard.
(1)Reduction of rope diameter below nominal
due to loss of core support, internal or
external corrosion or wear of outside wires.
(2)A number of broken outside wires and the
degree or distribution or concentration od
such broken wires.
(3)Worn outside wires.
(4)Sections of rope which are normally hidden
during inspection or maintenance
procedures, such as parts passing oversheaves, should be given close
inspection
as
these are points most subject to
deterioration.
(5)Corroded or broken wires at end
connections.
(6)Corroded, cracked, bent, worn or
improperly applied end connections.
(7)Kinking, crushing, cutting or unstranding.
(b)All rope which has been idle for a period of a
month or more due to shutdown or storage of a
hoist on which it is installed, shall be given a
thorough inspection before it is placed in service.
This inspection shall be for all types of deterioration
and shall be performed by an appointed or
designated person whose approval shall be required
for further use of the rope.  A written and dated
report of the rope condition shall be filed.
