Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees are exposed to:
...the hazard of being struck by the hoist load while moving loads
of tubing using a twin monorail hoist.  The hook on the south hoist
block was stretched greater that the 15 percent permitted by American
National Standards Institute's B30-10 Standard on hooks and the
safety throat latch was missing on both the south and north hoist
units permitting a spreader bar to become disengaged and fall free
on June 21, 1988.
Among other methods, one feasible method of abatement is to replace the
hook on the south hoist unit and to institute and inspection and
maintenance program to insure that any deficiencies to the hoisting
equipment are identified and repaired promptly.
