Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
...the hazards of being struck by falling loads and/or portions of
the hoisting boom and load line due to failure of side boom hoist
and/or load line components on the Caterpillar model number 572 side
boom tractor serial number 40V740, property number 16-9-018.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to perform a rated load test in accordance with
section 2.2.1(a) of American National Standards Institute standard B30.
14-1979 Side Boom Tractors, with the six part reeving provided in lieu
of the manufacturer's specified four part hoist line.

section hoist boom on the caterpillar model number 583 side boom
tractor serial number 61A943, property number 15-4-020.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to replace the damaged main and spare booms
for this unit with booms certified by the manufacturer as being the
equivalent of original equipment,  the required by section 2.3.3.(c)(3)
of Amerian National Standards Institute standard B30.14-2979 Side
Boom Tractors.

correct this hazard is to have all re-manufactured main and spare booms
for this unit with certification by the manufacturer as being the
equivalent of original equipment,  as required by section 2.3.1(b) of
American National Standards Institute standard B 30.14-1979 Side Boom
tractors.
