free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a fall hazard:
A) Plantwide - Employee(s) were being elevated on a wooden pallet
approximately 10 feet above the floor level, on the forks of a
powered industrial lift truck, on or about December 19, 1990.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to use an industrial lift designated as a manlift
with the appropriate safety provisions and a lifting platform which has
been designed for use on the forks of an industrial lift truck, with
standard railings and secured to the forks of the truck.
