Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
death and/or broken bones from a crushing injury by moving paint heads
or printing heads:
a) Printing Area - Employees disconnected the power to printing machine
number 1 by snapping an ordinary light switch, he then climbed under
the raised printing head, on or about December 19, 1989.
b) Printing Area - Employees climbed on printing machine number 1 while
machine head was moving in a vertical movement and paint head were
retracted, on or about December 19, 1989.
Among other methods one feasible and acceptable abatement method to
correct this hazard is have the machine locked out while service is
performed and kenetic energy sources blocked.
