Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) on 6/26/90 - employees were exposed to a falling hazard of 35 ft. while working on a horizontal wire rope supported scaffold system (catenary scaffold). The employees were exposed to a fall hazard by: 1) overloading the scaffold, 2) the scaffold was not equipped with hook stops, 3) safety belt lanyards were not attached to an independent line. Among other means of feasible and acceptable methods of abatement is to instruct employees in the design limitations of a catenary scaffold as stipulated by ANSI A10.8 1977, section 22 paragraph 22.4j. To provide hook shaped stops on each end of the staging memeber. As stipulated by ANSI 10.8 1977, section 22 paragraph 22.1 and to ensure the safety belt lanyard is secured to a member other than the scaffold as per ANSI 10.8 - 1977, section 22 paragraph 22.6.
