employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to multiple injuries from a fall:
(a)  Newberry Commons Site - Employee(s) exposed to a fall
hazard of  approximately 18 feet while removing metal
roof  decking  and were not provided with safety belt
and lanyard, or catch platforms, on or about November
25, 1992.
THE ALLEGED VIOLATIONS BELOW (1a 1b, AND 1c) HAVE BEEN GROUPED BECAUSE
THEY INVOLVE SIMILAR OR RELATED HAZARDS THAT MAY INCREASE THE POTENTIAL
FOR INJURY AND/OR ILLNESS.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide and wear safety belt and lanyard.
