Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to death because of poor signalling and verbal communication training for winch truck lifting operations: A) Field Office and Warehouse Yard Area - An employee was struck a strong blow to the chin by a skid when a winch truck pulled forward during lifting operations causing the grab hook to release from a partially lifted pipe due to poor communication, on or about July 28 and 30, 1990.
