Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a  place  of  employment  which
were free of   recognized hazards that were causing or likely to  cause
death or serious physical harm to  employees  in  that  employees  were
exposed to contact and inhalation hazards from leaks,  ruptures, and/or
releases from pipelines,  vessels,   and  equipment  handling  chlorine
dioxide.   The following condition contributed to the existence of this
hazard:
S. D. Warren Co., Skowhegan, Maine - The employer did not
conduct comprehensive, annual inspections of the west chlorine
dioxide storage tank by qualified personnel as recommended by
the manufacturer.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable method is to implement
an annual inspection program conducted by trained personnel, ex-
perienced in the design and fabrication techniques employed in this
equipment.
1.  Vessel identification means for in-plant identification.
2.  Documents necessary to record results of inspections,
repairs, and alterations.  Typically, this will include such
interior and exterior observations as cracking,
deterioration of the surface coat, presence of chemical
attack, and bondability.
