Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to respiratory injuries and asphyxiation from inhalation of chlorine gas that was released as a result of the accidental mixing of incompatible chemicals (alum and sodium hypochlorite): On or about 02/15/91 at 12 o'clock noon, Fraser Paper Mill, Bond Beater Room - The employer failed to ensure that a pipeline used to unload alum was properly connected to the alum storage tanks. Among other methods, one feasible and acceptable abatement method is to physically walk along the discharge line to ensure that it is properly connected to the appropriate storage tank.
