Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not

recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) Employees were exposed to a potential crushing hazard while working
near the
electric manlift on the main floor due to the lack of guardrails around
the base of the
electric manlift. Among other methods, one feasible and acceptable method
to correct this
hazard would be to install a standard guardrail system with an entrance
gate around the
base of the electric manlift.
(b) Employees were exposed to a potential hazard while operating the
electric manlift due
to the inoperable switch on the top floor. Among other methods, one
feasible and
acceptable method to correct this hazard would be to install a new switch
for
operating
the electric manlift.
(c) Employees were exposed to a fall hazard while operating the electric
manlift due to
the lack of a locking device on the elevator entrance door when it is in
operation. Among
other methods, one feasible and acceptable method to correct this hazard
would be to
install a locking device on the elevator car door to prevent it from
opening while in
operation.
