Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) Employees were exposed to a potential fall hazard while operating the
manually
operated manlifts due to the lack of complete car enclosures. Among other
methods, one
feasible and acceptable method to correct these hazards would be to
install a standard
guardrail which includes two rails on the following manlifts:
(1) The manlift in the west unit was not provided with a complete car
enclosure.
(2) The manlift in the main elevator was missing a midrail on the manlift
car.
(3) The manlift in the main elevator annex was enclosed on three sides but
had
no
guardrail gate on the entrance.
(4) The manlift in the pv house was missing a midrail on the manlift car.

(a) Employees were exposed to an impact hazard while pulling cars with a
single nylon
rope. Among other methods, one feasible and acceptable method to correct
this hazard
would be to change the method of moving cars by pushing or by using a
double nylon
rope system to prevent the hopper car from overcoming the tractor.
(b) Employees driving the tractor pulling cars were exposed to a potential
impact hazard
and being struck by the rope/hook in case it broke. Among other methods,
one feasible
and acceptable method to correct this hazard would be to install a guard
over the rear
window of the tractor and the use of a tag line on the nylon rope.
