Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to potential explosions when frozen manhole covers are removed by using propane torches and spark creating tools when the potential for an explosive atmosphere exists. One feasible method feasible among others to correct this hazard is to eliminate the use of torches as a means of removing the manhole covers and use other methods which eliminate ignition sources in and around the unopened manhole. A) LOCATION: 140 LAFORD TERRACE ON 2/8/94, WORKERS INVOLVED IN THE REMOVAL OF A FROZEN MANHOLE COVER WERE EXPOSED TO AN EXPLOSION WHEN PROPANE TORCHES AND SPARK CREATING TOOLS WERE USED ON AN UNOPENED MANHOLE WHERE AN EXPLOSIVE ATMOSPHERE EXISTED. CURRENT METHODS OF MONITORING DO NOT ENSURE THAT MANHOLES HAVE ATMOSPHERES BELOW THE LOWER EXPLOSIVE LIMIT BEFORE AND DURING THE COVER REMOVAL PROCESS WHERE IGNITION SOURCES ARE INTRODUCED.
