General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of inadequately labelled and maintained hoists and underhung
cranes:
Plant wide:  (1) The employer had not developed and implemented a
regular preventative maintenance and inspection program, including
written records, for all hoist and underhung cranes;
(2)  The following underhung crane and hoists deficiencies were
observed:
CONTROLS:  Each button on the pendant control for the hoist at the
Westinghouse Electric Furnace (BA area) was not labelled to indicate
its function.
SHEAVES:  Sheaves were worn on the following 2 ton hoists: #77, #70,
Rough Slitter, Strip cleaner pay-off.
WIRE ROPE:  Wire rope was damaged on the following hoists and
underhung crane:  #82 two ton hoist at Westinghouse Electric furnace (BA
area), Robertson-Meyer 3 ton hoist at finish end of furnace, five ton
underhung weigh-up crane in old casting shop.
Among other methods, one feasible and acceptable abatement method to
correct his hazard is to:
-Correct crane and hoist deficiencies that were observed during the
inspection .
-Ensure that maintenance personnel who perform periodic inspections and
maintenance on material handling equipment are thoroughly trained and
maintenance on material handling equipment are thoroughly trained and
are familiar with related consensus standards an manufactures
recommendations.
-Provide basic training regarding hoists, including sheaves, wire ropes,
braking systems and controls to employees who use the equipment on a
regular basis.  Employees could then perform daily visual inspections of
the equipment and report and damage or questionable equipment to the
maintenance supervisor instead of relying solely on periodic
preventative maintenance inspections to uncover possible unsafe
conditions,
-Maintain written records of preventative maintenance and crane
inspections.
