Executive Order 12196 of February 26, 1990, Section 1-201(j): The agency %% did not operate an Occupational Safety and Health Management Information System, which included the maintenance of such records as the Secretary required: a) Edith Nourse Rogers Veterans Memorial Hospital: On 8/1/89 the following information was not contained on the facilities summary log of injuries and illnesses as required: 1) The following cases were not logged when lost time was incurred: cases numbered 0181, 0219, 0058, 0051, 3162 and 2 cases with no log numbers dated 1/18/89 and 11/17/89. 2) The following cases in which medical expense was incurred: cases numbered 0158, 0161, 0219, 0282, 0051, 0379, 0313, 0271 and 3162.
