Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a fall hazard up to five stories:
a)  107-115 S. 2nd St. - Employees were not using lifelines
attached to a completely separate fall arresting system.
Observed 6/2/92
NOTE:  One useful means of abatement among other, would be to
install a life line completely separate fall arresting system and
being utilized by employees.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that there were no method being used
by the employer to determine if roof attachment points used to anchor the
primary support systems of "Sky Genies" were capable of supporting at least
5000 pounds:
a)  107-115 S. 2nd St. - Two employees engaged in window washing
activities using a "Sky Genie" were using iron railings to support
the suspension system.
NOTE:  One useful and feasible method of abatement, among, others
would be to load test the attachment points for the primary
support and lifeline prior to the start of the job.

engagged in window washing activities using "Sky Genie" systems were
exposed to a fall hazard of up to 5 floors since employees had not been
adequately trained in the use of this system:
a) 107-115 S. 2nd St. - Employees have not been adequately trained
in the use of "Sky Genies" in the employees were observed without
life lines attached to a completely separate fall arresting
system.  Observed 6/2/92
NOTE:  One useful and feasible means of abatement among other,
would be to ensure all window cleaners are trained in and familiar
with safety working from "Sky Genie" and be trained in the proper
safety use of the equipment in accordance with manufactures com-
mendations and OSHA Guildelines.

work from a "Sky Genie System":
a)  107-115 S. 2nd St., Phila, Pa. 19106 - Employees engged in
window cleaning activities were not protected by a pre-work rescue
plan for self rescue.  Observed 6/2/92
ABATEMENT NOTE:  One useful and feasible method of abatement among
others, would be to design and implement a rescue system which
does not rely on the suspended employee for rescue in the event an
accident occur.
