Executive Order 12196 of February 26, 1980 Section 1-02(a) - The head of the agency did not furnish to employees places and conditions of employment that were free from recognized hazards that were causing or likely to cause physical injury in that employees were exposed to the hazard of electrical shock while working in areas where 440 volt electrical cords are suspended by tie wire: a) U.S.S. Kitty Hawk, #4 MMR - a 440 volt 3 phase electrical service line cord was suspended by tie wire. Observed 03/26/90 Abatement NOte: Among other methods, one feasible and acceptable abatement method to correct this hazard is to use a non-conductive material to suspend 440 volt electrical cords.

Executive Order 12196 of February 26, 1980 Section 1-102(a) - The head of the agency did not furnish to employees places and conditions of employment that were free from recognized hazards that were causing or likely to cause physical injury in that employees were exposed to fall hazard while climbing ladders not equipped with a cage or ladder climbing safety device: a) North East Side of Drydock #5 Portal Crane #AL-101 - Crane operators and service shops employees who perform oiling functions to the sheave on the end of the boom are exposed to fall hazards while climbing ladder sections over 30 feet in length and over 30 feet above ground level that are not provided with any cage or ladder climbing safety devices. Observed 03/30/90. Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard would be to install a ladder climbing safety device on ladder sections located on the crane boom.
