Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
slips and falls when safe access was not provided when boarding tugboats
from the wharves:
a) Pier #40 - employees were not provided safe gangways, jacob's
ladders or other means to descend from wharves to vessel decks
and likewise ascend, especially during low tide.  Observed
01/24/92.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS...
To use a jacobs's ladder meeting the following requirements:
1. Jacob's ladders shall be of the double rung or flat tread
type.  They shall be well maintained and proplerly secured.
2. A Jocob's ladder shall either hang without slack from its
lashings or be pulled up entirely.
