Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to possible leakage, rupture, or failure of dryer cans containing steam and condensate. The following conditions contributed to the existence of this hazard: a. Paper Machines 1 and 2: A program of periodic inspections, and documentation of inspection results of dryers/dryer was last implemented and completed in the year 1987 or before. ABATEMENT NOTE: Among other methods, one feasible and accepted method is to implement an unfired pressure vessel program in accordance with TAPPI, 1988, TIS 040216, Paragraph E. Key elements of this five year program consist of the following: - A detailed internal and external visual inspection check conducted by experienced personnel. - magnetic particle testing and inspection of dryer heads conducted by trained and certified personnel. - Ultrasonic thickness testing measurements conducted by trained and certified personnel on any parts of the shell that appear to be thinned by corrosion or wear, as revealed by internal visual inspection. ABATEMENT SCHEDULE Step 1 Develop a written plan and submit it to the Area Director. Step 2 Complete the Abatement Plan and evaluate its effectiveness. Note: Each inspector shall have education and experience equal to at least one of the following: 1. A degree in engineering plus 1 year of experience in the design, construction, repair, operation, or inspection of boilers or pressure vessels. 2. A two year certificate in engineering or technology from a technical college plus 2 years of experience in the design, construction, repair, operation, or inspection of boilers or pressure vessels. 3. The equivalent of a high school education plus three years of experience in the construction, repair, operation, or inspection of boilers or pressure vessels.
