Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees:
a) High pressure explosions from natural gas wells that are constructed
using materials not approved or manufactured for the purpose in
which they were used and where the failure of such material could
cause the ejection of the materials from the well.
Among other possible methods of abatement the following is considered
the most feasible and reasonable to implement:
1) The use of a pressure tested Tension Wall Packer on natural
gas wells when an interior conduit (frac. pipe ) is used.
2) The installation of a short pipe nipple and coupling to
facilitate the pipe clamp, providing a pipe coupling below the
clamp to hold the pressure of the well in the event the packer fails.
3) The use of permanently attached bolts to the well casing to hold
the pipe clamp.
