Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to and/or crushing injuries from falling loads as a screw pin shackle was used without a key to prevent pin back out at the connection between the pipe hook and the wire rope sling: a) Alta Drive - The screw pin was backed out several turns on the shackle used to hoist a 20 foot section of eight (8) inch cast pipe. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THE HAZARD IS BY USING A SHACKLE WITH A KEY THRU THE SCREW PIN.
