Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazard of being crushed or pinned when permitted to operate the freight elevator, which is used to convey merchandise between the first and second floor, without the following safety measures in place. a) Electrical interlocks for the hoistway door on the first floor to prevent operation of the elevator from either the first or second floor with the hoistway door in the open position; On or about 8/1/90.
