free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the potential uncontrolled operation of the air hose/blow pipe operation
at approximately 130 p.s.i.:
a) Training Facility, Dam Neck, Building A, roof for east stairwell. A
compressed air blow pipe operating at approximately 130 p.s.i. was
not provided with a "dead man" control valve to protect employees
from the uncontrolled operation of the pipe.
Among other methods, one feasible and acceptable method to correct this
hazard is to install a "dead man" control valve.

ground and was not protected at all times with a safety belt and
lanyard. Among other methods, one feasibel and acceptable abatement m
ethod to correct this hazard is the use of a safety belt and lanyard whi
le working from these elevated work areas.
