General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling crane loads from a failed hook attachment (shackle): a) Employees use a P & H crawler crane (ss #15856) to lift and set piles. The lift load lines is fitted with an over haul ball with a hook shackled to the ball. The shackle is subject to eccentric loading thus exposing employees to injury from contact with falling material resulting from a failed shackle, 7/3/90. Among other methods, one feasible and acceptable abatement method to correct this hazard is to centralize the hook on the shackle pin with suitable washers or spacers.
