Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to being struck by dropped wads or falling hoists or
components:
a)  The Budgit 1500 pound air hoist in the Paint Shop had
the following conditions which contributed to the existence
of these hazards:
1)  The nut was not secured on the eye bolt from which the
hoist was suspended.
2)  The safety cable passed through the eyebolt, rather
than over or through the trolley assembly.
3)  No frequent or periodic inspections for damage or
deficiencies were conducted, on or about 3/4/92.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is:
1.  To secure the eye bolt and nut with a roll pin or a drive pin.
2.  To replace the restraining device (safety cable).
3.  To implement a program of frequent and periodic inspections,
with written, dated and signed reports on critical items.
"Frequent Inspections" are conducted on daily to monthly intervals and
includes daily inspection of all controls, operating mechanisms and
safety devices for malfunction, deterioration of air systems, daily
inspections of hooks and load-bearing components for damage, and visual
inspection daily of load-carrying ropes or chains for wear, etc.,
improper dead-ending to the hoisting drum and other attachments, and
for deposits of material which could be carried into the hoist.
"Periodic Inspections" are conducted on one to twelve month intervals
and covers the items checked "frequently" plus:
a)  Loose bolts or rivets
b)  Cracked or worn drums or sheaves.
c)  Worn or damaged pins, bearings, shafts, gears, rollers,
locking and claming devices.
d)  Excessive wear on motor or load brake.
e)  Excessive wear of chain, rope, load sprockets, drums,
sheaves, and chain stretch.
f)  Hooks - crack detecting inspections at least one a year.
g)  Electrical apparatus for pitting or deterioration (controller
contractors, limit switches, push button stations).
h)  Hook retaining nuts or collars and their pins, welds or
riveting.
i)  Supporting structure and trolley for ability to support the
load.
j)  The presence of the warning label.
