General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a fall hazard:
a)  Canada Dry Bottling Company of Lehigh Valley, Bethlehem,
Pennsylvania - An eployee was exposed to an approximate
twelve (12) foot fall hazard while using a work platform
that was elevated by a Clark forklift for the purpose of
installing metal racking in the warehouse area.  The work
platform was placed on the forks of the truck and it was
not secured to the forklift and lifting carriage and/or
forks.  Also, the work platform's top rail height was
approximately 27 inches above the surface of the platform
and one side of the platform's railing was not installed
with a midrail, on or about October 29, 1992.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to install and use safety platforms in
accordance with the "safety standard for low lift and high lift
trucks" (ANSI B 56.1-1988).
