Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
fire and explosion hazard:

a) Employees were assigned and directed to perform hot
work (welding) where flammable or combustible liquids and
vapors were present inside the system being welded upon,
on or about October 3, 1991.

Among other methods, one feasible and acceptable abatement method to
correct this hazard is to develop hot work procedures and safe welding
procedures for use when welding is to be done on lines containing
hazardous substances.  Such procedures should outline steps to be taken
from set-up throough the welding operation following guidelines set
forth in American National Standards Institute Z49.1-88 "Safety in
Welding and Cutting", National Fire Protection Association Standard 51B
Fire Prevention in Use of Cutting and Welding Processes" and American
National Standards Institute/American Welding Society Publication
F4.1-8, "Recommended Safe Practices for the Preparation for Welding
and Cutting of Containers That Have Held Hazardous Substances".
