Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a 30 foot fall hazard:
a)  Service Tower Southeast Corner - The employee positioned
the boom and work platform of the JLG lift over two (2)
employees working on a two point suspended scaffold and
within inches of the outrigger beam, exposing the
employees to a 30 foot fall hazard, on or about
April 24, 1992.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to instruct the employee in the safe
operating practices of the lift.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a 16 foot fall hazard:
a)  Building Three (3), Penthouse - In February, two
employees were working on a two point suspended scaffold
16 feet above the roof.  The 3/4 inch shackle had the pin
removed and replaced be a bolt.  The nut did not have a
key to prevent it from loosening.  The third were rope clip
was pushed up two (2) inches by two-blocking the hoist.
The metal counterweights were chained together but not
secured to the outrigger beam.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to follow the manufacturers specifications
and irect the scaffold properly.
