Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a 11 foot 11 inch fall hazard from a missing mid-rail chain: a) King of Prussia Mall Column B29 - The mid-rail chain was missing on the platform end of the condor self propelled aerial lift serial number X835-1564. The employer was elevated 11 feet 11 inches high, on or about May 17, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard maintain railing on the platform in accordance with the American National Standards Institute A92.6 Self-Propelled Work Platforms.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a 15 foot fall hazard: The alleged violations below have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident. a) King of Prussia Mall, Second Floor - Two (2) employees were spreading bar joints and tack welding them in place. They also laid decking and were exposed to a 15 foot fall hazard to concrete floor and debris below, on or about May 17, 1992 Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide fall protection for employees.
