Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a foot injury: a) Bonney Forge - Numerous powered chain hoists, such as the CM series 635 electric chain, one (1) ton, used to hoist molds weighing up to 600 pounds four (4) feet off the floor and place them on a work or milling table and no inspection program was implemented, on or about February 2, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to institute a inspection program as outlined in ANSI B30.16.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a 12 foot fall hazard while working from an unsecured and unguarded work platform being elevated by a forklift: a) Die Shop - The employee was working off an elevated platform that was not firmly secured to the forks of the powered industrial truck and no railing was provided on a three (3) foot opening, on or about February 2, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: a. Use a safety platform firmly secured to the lifting carriage and/or forks. b. Provide fall protection with a safety belt and lanyard or place chain across the three (3) foot opening.
