Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or were likely to cause death or serious physical harm
to employees in that
employees were exposed to a crushing hazard in the event of a speed loader
crane component
failure.
a) Worthington Steel, Malvern PA - the speed loader crane had a badly worn
lower load sheave. This could cause the sheave to fail causing the speed
loader
to loose its load causing injury to the employee on the ground guiding
loads
being landed.  Observed 05/31/95.
ABATEMENT NOTE:  One feasible and useful means of abatement would be
to perform frequent (monthly) inspections of the speed loader components
and
repair or replace deficiencies.
