General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling hazards: a) Blast Department - A portable fiberglass ladder was improperly used to enable employees to access the control room from the charge shed area. The ladder was not placed on secure footing as rubble was strewn all around the base, it was not secured to prevent tripping over and the employees had to climb over standard railing in order to access the floor level above or egress therefrom. Observed 02/28/96.
