a) Blast Department - A portable fiberglass ladder was improperly used to
enable
employees to access the control room from the charge shed area.  The
ladder was
not placed on secure footing as rubble was strewn all around the base, it
was not
secured to prevent tripping over and the employees had to climb over
standard
railing in order to access the floor level above or egress therefrom.
