Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to hazardous conditions posed by faulty
equipment:
a) Maren 122 Baler - The master control hydraulic locking
device of the Maren 122 Baler, which controls the release
of the trash baler door was not maintained in operative
condition.  A metal rod was routinely used by employees
to pry open the door after a completed bale had been
tied.  Observed 09/24/96.
b) Maren 122 Baler - A mechanical safety feature of the
Maren 122 closed chamber automatic high density baler was
not functioning.  The "Full Bale" warning buzzer (which
was designed to take the guess work out of the baling
process and to achieve maximum density in each bale) had
been disabled.  Observed 09/24/96.
