Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees were exposed to:
a)  Hazard of being caught in moving machinery, in that the lockout
keys are kept on chains, immediately adjacent to the control panels
serving the 3 tile lines.
Among other methods, one feasible and acceptable abatement method t
o correct this hazard is to develop and implement a lockout/tagout
plan that should include but not be limited to, the following:
1)  Job objectives and involved equipment/process
2)  Estimated job duration
3)  Personnel and crafts involved
4)  Personnel sign-on and off provisions
5)  Type, number, and location of all energy isolating devices requ
iring lockout/tagout devices
6)  Responsible personnel approvals
7)  Start-up provisions
8)  Relocation of the lockout keys from the immediate area, where t
he locks are located, when the machinery is locked out.
