General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to the hazards of crushing, concussions, death and/or multiple traumatic injuries during the operation of the C & M Lodestar Hoist or as a result of crane beam failure. Conditions noted at the time of the inspection which contributed to this hazard are as follows: (a) The "I" beams (runways) were not secured to the building structure (concrete girders) braced and supported to prevent the runways movement and to reduce fatigue and bowing problems. (b) The employees used to Columbus-McKinnon one ton Lodestar electrically operated chained hoist (under hung hoist) to raise, lower and position steel vats of ink loads with an approximate over all weight of eight hundred pounds. (c) The non-attachment of the "I" beams would allow the load to become unbalanced and thereby cause a twisting of the "I" beams, upsetting the load and allowing the load to strike employees. Among other methods, one feasible and acceptable abatement method to correct this hazard is to include, but not be limited to the following: (a) Analysis of the general construction of the building structure, the runways (I beams), supporting girders and other crane support structure shall be thoroughly checked by a qualified structural engineer in accordance with the American National Standard of Monorails and Underhung Cranes ANSI/ASME B30.11-1988, ASME/ANSI B30.16 for Trolleys and ANSI MH 27.1 The Tracks (Beams) Supports per the manual of The American Institute of Steel Construction.
