General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  Employees were exposed to being struck by failed hoist
loads when charging the number one reverb furnace due to
damage of the hoist load line which had 20 - 30 broken
wire rope strands in a one foot section of one rope lay.
(b)  Employees were exposed to being struck by failed
hoist loads when transferring compressed scrap from the
presses to pallets and when hoisting various items in the
welding shop, maintenance shop, and other locations where
these small hoists are utilized in the plant.
Among other methods, one feasible and acceptable method
of abatement to correct this hazard is to initiate a inspection
and replacement program such as that outlined in American
National Standards Institute B30.11 "Monorail Systems and
Underhung Cranes" - 1973 which include, as a minimum,
the following elements:
(1)  Visual inspection of all running ropes once each
working day.
(2)  Thorough inspection of all ropes at least monthly
with a full written, dated and signed report of rope
conditions kept on file.
(3)  Removal from service or ropes with any
deterioration resulting in appreciable loss of original
strength where further use of the rope would
constitute a safety
