Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of being struck by falling loads being lifted
by a truck crane.
The alleged violations below have been grouped because they involve
similar or related hazards that may increase the potential for injury
resulting from an accident.
The load of bar joists being hoisted was rigged in such a manner
that the wire rope sling pulled against one leg of the shackle
which was attached to the headache ball.  Applying pressure to
a leg of the shackle could lead to the legs opening up and
dropping
the load.
Among other methods, one feasible and acceptable means to correct
this hazard is to follow recommended safety practices in the
attachment of slings to the hook of the crane.
