were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On or about March 25, 1992, employee(s) were using the
Roto-Loft lift Model #RL48-23, Serial #RL211 were exposed to the
hazard of fracture, etc., from possible undetected defects.
One feasible and acceptable method to correct this hazard is
the implementation of a program of inspections and maintenance in
accordance with the manufacturer's operating/maintenance manual.
